Death of 18-Year-Old Passenger on Carnival Horizon Classified as Homicide

The death of Florida teenager Anna Kepner, discovered aboard the Carnival Horizon earlier this month, has been officially ruled a homicide, according to a death certificate released to the family and reviewed by news outlets on Monday. The document lists the cause of death as “mechanical asphyxia” and states that the 18-year-old “was mechanically asphyxiated by other person(s).” The date of injury is recorded as Nov. 6; the time remains unspecified.

Kepner was traveling with her father, stepmother, grandparents, siblings and stepsiblings on a week-long Caribbean itinerary when she was reported missing. A shipboard search led security personnel to a cabin where the teenager was found under a bed, wrapped in a blanket and covered with life vests, according to a source briefed on the investigation. The vessel was at sea between ports when the discovery was made.

Investigators notified the family that preliminary observations pointed to asphyxiation, possibly resulting from a “bar hold,” a maneuver in which an arm is pressed across the neck. That detail, conveyed during initial briefings with relatives, aligns with the formal determination on the death certificate.

The Federal Bureau of Investigation, which has jurisdiction over serious crimes on U.S.-flagged cruise ships, has not commented publicly on the case. A spokesperson for the Miami-Dade Medical Examiner’s Office also declined to discuss the findings, citing the ongoing nature of the inquiry. Autopsy and toxicology reports have not yet been released.

While no criminal charges have been filed, a court document in an unrelated family proceeding indicates that Kepner’s stepbrother, a minor, could become the focus of potential charges. Authorities have not confirmed whether the teenager is currently considered a suspect, nor have they detailed any investigative steps taken since the ship returned to its home port in Miami.

The cruise line has said it is cooperating fully with federal investigators. Standard protocols for onboard deaths typically include sealing the cabin, preserving relevant evidence and providing closed-circuit television footage to law enforcement. It remains unclear how much CCTV coverage exists of the corridor and cabin area where Kepner was found.

According to family members, Kepner had been an honors student preparing to graduate from high school in May. She had expressed interest in joining the U.S. Navy upon completion of her studies. Relatives have described her as independent and forward-looking, emphasizing the abrupt loss of what they considered a promising future.

Mechanical asphyxia occurs when external pressure prevents normal breathing, inhibiting oxygen flow to the lungs and brain. Forensic experts note that signs of such asphyxia can include neck abrasions, petechial hemorrhages and internal tissue damage, details that are typically documented in full autopsy reports. Those reports, once complete, are expected to provide additional information on the exact mechanics of Kepner’s death.

In homicide investigations on cruise ships, cooperation among multiple agencies—ship security, local port authorities, medical examiners and the FBI—is common. The complexity is heightened by overlapping jurisdictions and the need to reconstruct events that take place in confined quarters while the vessel moves between international waters and various territorial boundaries.

The Carnival Horizon has since resumed regular operations. Passengers booked on subsequent sailings were informed only that a medical emergency had occurred; no itinerary changes were announced. Carnival representatives have not disclosed whether any crew members or passengers have been restricted from travel while the investigation proceeds.

Kepner’s remains were transported to Miami shortly after the ship docked on Nov. 9. Funeral arrangements are pending, with the family stating they will wait for the full investigative file before finalizing plans. They have called for a thorough examination of all evidence and have asked the public to respect their privacy during what they describe as a period of intense grief.

Authorities have provided no timeline for possible arrests or additional disclosures. The case remains active, and forensic results, when completed, are expected to clarify circumstances surrounding the teenager’s final hours.
